What are the Requirements?
1)   Articles of Incorporation
a) notarized copies; each copy should be duly signed by the Board of Directors.
2)   By-Laws
a) 5 notarized copies; each copy should be duly signed by the Board of Directors.
3)   Registration Data Sheet (5 copies)
4)   Verification Slip (2 copies)
5)   Written undertaking to change corporate name (5 copies)
6)   Treasurer's Affidavit (5 copies)
7)   Bank Certificate of Deposit (8 copies)
8)   Authority to Verify Account (5 copies)

What are the Requirements?
1)   Endorsement from the assisting agency and Provincial/Municipal Cooperative Specialist
2)   Economic Survey
a) Accomplished Simplified Economic Survey Form for Primary Cooperatives
b) Economic Survey Form for special types of coops, i.e. Cooperative Banks and Electric Cooperatives, with at least three (3) years financial Projection

  • Balance Sheet
    3)   Articles of Cooperation
    a) Number of Board of Directors, not less than 5 but not more than 15, always in odd number and not related up to 3rd degree of consanguinity and affinity
    b) Directors, Secretary and Treasurer included as shareholders
    c) Authorized Capital
    d) Subscribed Capital, at least 25% of the Authorized
    e) Paid up Capital, at least 25% of the Subscribed and should not be less than Php 2,500.00
    f) Signed by all the cooperators
    g) Documents Subscribed and sworn to before a Notary Public
    h) Adopted in the presence of two (2) witnesses who are not cooperators
    4)   By-laws
    a) Mandatory Statutory Provisions
  • Reserve Fund - 10% minimum
  • Education and Training Fund - 10% maximum
  • Optional Fund - 10% maximum
    b) Majority of the members must sign in the adoption of By-laws
    c) Signed in the presence of two (2) witnesses who are not members

    Single Proprietorship
    1)   Registration fee of Php 300.00
    2)   Two (2) copies 2 x 2 recent pictures of owner
    3)   Three (3) alternative names aside from the preferred name
    4)   Three (3) pieces documentary stamps
    5)   Application forms duly filled-up and signed by owner

    Cooperative, Corporation & Partnership
    1)   Registration fee of Php500.00
    2)   SEC Certificate (photocopy)
    3)   Articles of Cooperation/Incorporation (photocopy)
    4)   By-laws (photocopy)
    5)   Board Resolution authorizing signatory for and in behalf of the cooperative/corporation/partnership
    6)   Documentary stamps (Php 15.00)
    7)   Two (2) copies 2 x 2 pictures of authorized signatory (optional)
    8)   Application forms fully filled-up and signed
    9)   If an alien, he must submit the following:
    a)   Alien Certificate of Registration;
    b)   Accomplished DTI Form No. 17 under R.A. 7042;
    c)   Written Appointment of Filipino Resident Agent Authority;
    d)   Authority to verify bank accounts/bank certificates of bank deposits;
    e)   Proof of inward remittance of foreign currency for non-resident alien and Bank Certificate of Deposit for resident alien;
    f)   Copy of valuation report from Central Bank if investment includes assets other than foreign exchange;
    g)   Certification from resident alien not seeking remittance of profit or dividends;
    h)   Clearance from other involved agencies as Dept. of Science & Technology, Philippine National Police, etc.;
    i)   In case of alien retailer, latest permit to engage in retail business per R.A. 1180; and
    j)   If corporation/partnership, SEC Certificate of registration & Certificate of Authority from SEC.

    What are the Requirements?
    1)   Applicant is required to submit the following:
    a)   Duly accomplished application form, Form 501;
    b)   If corporation or partnership, SEC Certificate of Registration and Articles of Incorporation/Partnership;
    c)   If single proprietorship, certificate of Business Name with the DTI;
    d)   For corporations, company's Board Resolution authorizing an officer to sign for and in behalf of the applicant enterprise;
    e)   Two (2) copied of project report and supporting documents; and
    f)   Proof of financial capacity, Income Tax Returns and/or Sworn Statement of Assets and Liabilities.
    2)   Fees
    a)   Project costs not exceeding Php 4 million - Fee: Php 1,500;
    b)   Project costs exceeding Php 4 million but NOT over Php 20 million - Fee: Php 3, 000.00;
    c)   Project costs exceeding Php 20 million but not over Php 50 million - Fee: Php 4,500.00; and
    d)   Project costs exceeding Php 50 million - Fee: Php 6,000.
    3)   Publication of Notice of Acceptance of Application
    4)   Deliberation by the Board/Processing within 20 working days
    5)   Approval
    6)   Submission of Pre-registration requirements and signing of Terms and Conditions
    7)   Payment of Fee for Registration Certificate
    a)   1/10 of 1% Project Cost, but not less than Php 3,000.00 and not more than Php15,000.00; and
    b)   Issuance of Registration Certificate.

  • What are the Requirements?
    For Corporations:
    1)   Securities and Exchange Commission (SEC) Registration;
    2)   Articles of Incorporation and By-laws;
    3)   Annual Registration fee of Php500.00;
    4)   Mayor's Permit;
    5)   Accomplished BIR Form 1903 (TIN will be issued upon completion of these requirements); and
    6)   If TIN has been issued, accomplish BIR Form 2000 and pay the Documentary Stamp Fee after assessment (would vary according to the value of subscribed capital stock).

    For Cooperatives:
    1)   CDA Registration;
    2)   Articles of Cooperation and By-laws; and
    3)   Exemption Certificate.

    What are the Requirements for Securing an Environment Compliance Certificate?
    1. Initial Environmental Examination (IEE) or environmental feasibility study
      1. Site Development Plan showing all facilities
      2. Vicinity map adequately identifying the location or premises of project and the nearby land uses establishment
      3. Pictures of establishment and anti-pollution devices
      4. Anti-pollution designs and plans
    2. Business Regulatory Documents
      1. DTI Registration and/or (if applicable)
      2. SEC Registration
    3. Zoning/Locational Clearance issued by:
      1. the Housing and Land Use Regulatory Board; or
      2. the Deputized Administrator (MPDC of the Municipal Government Unit where the project is situated)
    4. Certification from the Local Government Unit that it has consulted its constituents and that they interpose no objection whatsoever against the project ("Barangay and Municipal Clearance Resolution")
    Note: All documents required shall be submitted in three (3) legible copies and placed in a folder

    Investment Simplified Process to Expedite and Effect Documents (I-SPEED)

    The Applicant's Authorized Representative can apply for the Business Permit
    New Business Permits are assessed based on the initial investments while Renewals are based on the Annual Gross Sales of the preceding year
    Schedule of Fees are based on the Local Tax Code
    Permit is valid until the end of the year.
    Renewal Period is from January 2 to January 20 of the following year
