First 100 Bulacan media practitioners to receive Philhealth cards

CITY OF MALOLOS – The Provincial Government of Bulacan (PGB) thru the initiative of Gov. Wilhelmino M. Sy-Alvarado will give out free Philhealth Insurance cards to first 100 media in Bulacan as part of the security program of PGB, which can be used in times of sickness and emergencies.

In order to avail of the Philhealth program, applicants must secure and submit the necessary requirements such as filled-out application form (available at the Provincial Public Affairs Office), marriage contract for those who are married, birth certificate of children ages 21 and below, valid I.D, and if the applicant has a senior citizen parent, submit the birth certificate of the applicant instead.

After the registration, the submitted documents will be brought to the Provincial Social Welfare and Development Office (PSWDO) for evaluation.
The first 100 Bulacan media practitioners who were able to submit the required documents and passed the PSWDO evaluation will be awarded the free Philhealth card.

Gov. Alvarado said, “malaking kagaanan ito para sa mga kalalawigan nating mamamahayag na madalas ay nasa field at bantad sa disgrasya at sakit, sa tulong nito, magkakaroon sila ng katuwang”.
Meanwhile, a strict policy is being implemented wherein only one media practitioner per family can apply for the said program to give equal opportunity for other aspiring applicants.

Interested applicants may apply on April 18-27, 2011 at the PPAO.
